Asked how long ago he was told, Sheen said: "Roughly four years ago."
"I thought I had a brain tumour, I thought it was over."
In a letter to Matt before the interview, he said: "I chose or hired the companionship of insipid types.
"A deluge of blackmail and extortion took center stage in this circus of deceit."
He admitted to paying people off and spending "millions" to keep them quiet, although he couldn't remember exactly how many and it was money meant for his children.
Sheen added: "It's a hard, three letters to absorb, it's a turning point in one's life."
The actor admitted he was doing a lot of drugs, making bad decisions and was depressed but said it was "impossible" that he has passed on HIV to anyone else.
He revealed he has had unprotected sex but spoke to doctors and his partners and they knew about his diagnosis well ahead of time.
Sheen added that he didn't know entirely how he had contracted the disease.
The controversial star, wearing a smart dark suit, white shirt and blue patterned tie, looked worried but managed a smile as the camera panned over to him in the studio.
NBC News announced on Monday that Sheen would make a "revealing personal announcement" in an exclusive, live interview after his reps declined to comment on the rumours for 18 months.
On their website, NBC said the Anger Management actor would "appear live in Studio 1A."
